Please do not submit patient names, dates of birth, diagnoses, or any other health information through this website. Patient referrals are accepted only by fax at (775) 244-1998 or by phone at (702) 747-8027, through systems covered by HIPAA business associate agreements. Any health information submitted through a web form in error will be deleted.
When you fill out a contact or referral-inquiry form, we collect the information you provide: your name, organization, role, phone number, email address, and message. We also collect basic technical information automatically, such as browser type, device type, pages visited, and approximate location, through standard analytics tools and cookies.
We do not sell your personal information. We do not share it with third parties for their own marketing.
This website and its forms are hosted and processed by third-party service providers, including our website and customer-relationship platform, who process information on our behalf and under our instructions. These providers are not authorized to use your information for any other purpose.

We keep contact-form submissions for as long as needed to respond to and maintain our relationship with referral partners, and then delete or anonymize them.

This website is intended for adults and healthcare professionals. We do not knowingly collect information from anyone under 18.
Patients who receive care from HarmonyCare are protected by HIPAA. Our Notice of Privacy Practices, which describes how we use and protect your medical information, is provided to patients at the start of care and is available on request by calling (702) 747-8027.
